ShiftKey Notarization Process and Requirements
The ShiftKey notarization process involves starting the signing on the Proof platform, verifying identity with a valid government-issued photo ID and an address ID matching Form 1583, connecting via live video with a commissioned notary who witnesses the signature, and then completing payment to download the notarized documents, with specific acceptable forms of ID including passports, driver’s licenses, and various official documents.
How it works
1. Start the signing process
ShiftKey and Proof have pre-loaded your document into the Proof platform. When you're ready, scroll to the bottom of the page and click the blue "Notarize now" button to get started.
2. Verify your identity
Proof will ask you a series of questions and prompt you to provide photos of a valid, government-issued ID to quickly and securely verify your identity.
3. Connect with a live notary
Proof will connect you with a commissioned notary over a live video feed. The notary will confirm your identity, witness your signature, and assist you throughout the process.
4. Download your documents
After the notary meeting, Proof will prompt for credit card payment before you can access your completed forms. You’ll then be able to download your documents and send them to their final destination.
Here's what you need
ID #1: The Photo ID
The Photo ID must be unexpired with a clear photograph. Acceptable Photo IDs include:
- State-issued Driver’s or Nondriver’s Identification Card
- Passport
- U.S. Permanent Resident Card issued by U.S. Citizenship and Immigration Services
- U.S. Certificate of Citizenship or Naturalization
- Matricula Consular (Mexico)
- NEXUS Card (Canada)
Please note: Customers without a Social Security Number are required to notarize through the online notary and must use a Passport as their Photo ID.
ID #2: The Address ID
The Address ID must match the address listed on Form 1583 (4f-j and 9b-f). Acceptable Address IDs include:
- State-issued Driver’s or Nondriver’s identification Card (Unless used for Photo ID)
- Voter or Vehicle Registration Card
- Home or Vehicle Insurance Policy (Declarations page is suggested)
- Lease, Mortgage, or Deed of Trust (*See note below)
- Leases: include name, address, start, and ending date, and signature pages.
- Deeds: include name, address, and signature pages.

Additional Scenarios
Single Participants
Form does not require spousal consent.
Married Participants
Plan participants should download the spousal waiver form and have their spouse complete this process.
Single Signer
For forms that require one signer, or multiple signers who are not in the same location.
Multiple Signers
For forms that require multiple signers who can meet with the notary from the same location.
General Requirements
- A single PDF that includes all form pages
- A valid form of ID
- The last four digits of your U.S. Social Security Number
- A credit card to pay the $25 notarization fee.
