Transamerica Notarization Process with Proof

The Transamerica notarization process with Proof involves uploading an unsigned PDF form, verifying identity with a valid government-issued photo ID and matching address ID, connecting via live video with a commissioned notary who witnesses the signature, and completing payment before submission, with Transamerica processing requests within three business days and no need to mail or fax the form.

How it works

  1. 1.

    Upload your form

    • Collect all pages of your form as a single PDF. You can fill out the form before uploading or complete it on the Proof platform. Do not sign the form before uploading; you must sign during the notary session.
  2. 2.

    Verify your identity

    • Proof will ask you a series of questions and prompt you to provide photos of a valid, government-issued ID to verify your identity.
  3. 3.

    Connect with a live notary agent

    • Proof connects you with a commissioned notary over a live video feed. The notary confirms your identity, witnesses your signature, and assists you throughout the process.
  4. 4.

    You're finished!

    • After the notary meeting, you will be prompted for credit card payment before your completed form is submitted to Transamerica for review. Transamerica typically processes requests within three business days. Do not mail or fax a copy of this form to Transamerica.

Here's what you need

ID #1: The Photo ID

The Photo ID must be unexpired with a clear photograph. Acceptable Photo IDs include:

  • State-issued Driver’s or Nondriver’s Identification Card
  • Passport
  • U.S. Permanent Resident Card issued by U.S. Citizenship and Immigration Services
  • U.S. Certificate of Citizenship or Naturalization
  • Matricula Consular (Mexico)
  • NEXUS Card (Canada)

Note: Customers without a Social Security Number must notarize online and use a Passport as their Photo ID.

ID #2: The Address ID

The Address ID must match the address listed on Form 1583 (4f-j and 9b-f). Acceptable Address IDs include:

  • State-issued Driver’s or Nondriver’s Identification Card (unless used for Photo ID)
  • Voter or Vehicle Registration Card
  • Home or Vehicle Insurance Policy (Declarations page suggested)
  • Lease, Mortgage, or Deed of Trust (*See note below)

*Leases: include name, address, start and ending date, and signature pages. *Deeds: include name, address, and signature pages.

Additional Requirements

  • A single, unsigned PDF that includes all form pages
  • A valid form of ID
  • The last four digits of your U.S. Social Security number
  • A credit card to pay the $25 notarization fee

If your form requires a medallion signature guarantee: Transamerica will accept a notarization through Proof as an alternative.

If your form requires a signature from your employer or plan administrator: Your form cannot be submitted through Proof. You must obtain the additional signatures and submit the form to Transamerica.

Participant Types

Single Participants

Form does not require spousal consent.

Married Participants

Plan participants should download the spousal waiver form and have their spouse complete this process.

Signer Types

Single Signer

For forms that require one signer, or multiple signers who are not in the same location.

Multiple Signers

For forms that require multiple signers who can meet with the notary from the same location.
