Wright-Patt Credit Union Notarization Process
The Wright-Patt Credit Union notarization process involves uploading an unsigned, single-PDF form, verifying identity with a valid government-issued photo ID and matching address ID, connecting via live video with a commissioned notary who witnesses the signature, and completing payment before accessing the notarized documents, with specific instructions for multiple signers and ID requirements detailed.
How it works
1. Upload your form
Collect all pages of your form as a single PDF. You can fill out the form before uploading or complete it on the Proof platform. Do not sign the form before uploading; you must sign during the notary session.
If your form requires more than one signer, both must be present in the same room to meet with the online notary. For multiple signers who cannot meet the notary together, refer to the provided directions. Note that this method incurs the highest total fee due to separate notary sessions.
2. Verify your identity
Proof will ask you a series of questions and prompt you to provide photos of a valid, government-issued ID to quickly and securely verify your identity.
3. Connect with a live notary
Proof will connect you with a commissioned notary over a live video feed. The notary will confirm your identity, witness your signature, and assist you throughout the process.
4. You’re finished!
After the notary meeting, Proof will prompt for credit card payment before you can access your completed forms.
Here's what you need
ID #1: The Photo ID
The Photo ID must be unexpired with a clear photograph. Acceptable Photo IDs include:
- State-issued Driver’s or Nondriver’s Identification Card
- Passport
- U.S. Permanent Resident Card issued by U.S. Citizenship and Immigration Services
- U.S. Certificate of Citizenship or Naturalization
- Matricula Consular (Mexico)
- NEXUS Card (Canada)
Note: Customers without a Social Security Number must use a Passport as their Photo ID and are required to notarize through the online notary.
ID #2: The Address ID
The Address ID must match the address listed on Form 1583 (4f-j and 9b-f). Acceptable Address IDs include:
- State-issued Driver’s or Nondriver’s Identification Card (unless used for Photo ID)
- Voter or Vehicle Registration Card
- Home or Vehicle Insurance Policy (Declarations page suggested)
- Lease, Mortgage, or Deed of Trust
Leases must include name, address, start and ending date, and signature pages. Deeds must include name, address, and signature pages.

Single Participants
Form does not require spousal consent.
Married Participants
Plan participants should download the spousal waiver form and have their spouse complete this process.
General requirements
- A single PDF that includes all form pages
- A valid form of ID
- The last four digits of your U.S. Social Security number
- A credit card to pay the $25 notarization fee
Single Signer
For forms that require one signer, or multiple signers who are not in the same location.
